Elderberry's reputation is "boosts immunity" — the actual clinical trials tested something much narrower.
Elderberry (Sambucus nigra) contains anthocyanins with antiviral and mild anti-inflammatory activity, thought to interfere with how certain cold and flu viruses attach to and replicate in respiratory cells — a treatment-during-illness mechanism, not a prevention mechanism.

What the trials actually found
Randomized controlled trials using doses of 175-600mg per day found elderberry extract shortened cold and flu symptom duration by up to four days compared to placebo in some trials. In an air-traveler study specifically, the placebo group experienced 117 total symptom-days compared to 57 in the elderberry group.1
Where the evidence gets weaker
Evidence for prevention — actually reducing the risk of catching a cold or flu in the first place — is much weaker and inconsistent across trials. One placebo-controlled trial found no benefit for flu duration or severity at all.2 The overall evidence base remains mixed, with researchers calling for more trials.
Why the distinction matters
Elderberry is usually marketed as a daily "immune boost" to take proactively. The strongest trial support is actually for a narrower use: taking it after symptoms already start, not as an ongoing preventive supplement.
