Most People Taking Fish Oil Aren't Taking Enough of It to Do Anything

"Eat more fatty fish for inflammation" is common advice. What's usually missing is how much fish — or how much EPA and DHA specifically — the research behind that advice actually used.

EPA and DHA aren't interchangeable

Fatty fish provides two distinct omega-3 fatty acids: EPA and DHA. EPA in particular converts into resolvins and protectins — signaling molecules that actively resolve inflammation, rather than simply blocking inflammatory pathways the way some anti-inflammatory drugs do.1

The dose threshold

A 2025 systematic review and meta-analysis found that doses of 1-3 grams per day of combined EPA and DHA produced the most consistent reductions in inflammatory markers — CRP, TNF-alpha, and IL-6.2 Below that range, results across trials were inconsistent. Most public health guidance for general omega-3 intake sits well under this therapeutic range, which is part of why casual fish consumption often doesn't move the needle on measured inflammation.

How long it actually takes

Meta-analyses show measurable changes in inflammatory biomarkers typically begin appearing after 3-8 weeks of consistent intake at that dose, with peak effects around 3-6 months.3 This is a build-up effect, not something that shows up after a single fish dinner.

The practical takeaway

  • A single weekly salmon fillet, averaged across the week, usually falls short of the 1-3g/day range used in the strongest trials
  • Fatty fish (salmon, sardines, mackerel) 2-3 times a week gets closer to the effective range than an occasional fillet
  • Consistency over months, not occasional intake, is what the actual research measured
